What are Dr. Shatkin's minimum requirements to review my case?

The lab prescription form must be filled out completely. Don't forget the shade!

You may email digital images to xrays@shatkinfirst.com and Dr. Shatkin will respond to your recommendations. Please make sure that the digital image is in JPEG format.
